How to Request a North Carolina Crash Report from the DMV

Crash reports investigated by all of North Carolina’s law enforcement agencies are forwarded to the North Carolina Division of Motor Vehicles.

Here is how you can obtain a copy of your report in person, phone, or mail.

How to Obtain a Copy of a North Carolina Crash Report

There are three ways you may obtain copies of crash reports from the Division of Motor Vehicles in North Carolina.

The Division of Motor Vehicles will provide certified copies of crash reports for $5.50. Payment can be made by credit card or check/money order made payable to NCDMV.

  • By Mail

When applying for a copy of a crash report by mail, you must first complete the TR-67A form and mail it with a check/money order to:
